Spreadsheets are the default tool for rehab tracking in most organisations. They're familiar and flexible. That flexibility is exactly what makes them unreliable as a place to hold rehab data.
Where Spreadsheets Break Down
A spreadsheet is a grid of cells, not a data system. It has no concept of an "objective," a "criterion," or an "athlete" as a defined object, only text typed into a box however the person filling it in chose to type it.
That creates a handful of specific, compounding problems. The same objective gets typed differently by every practitioner who enters it, with nothing linking those entries as the same thing. Nothing validates what goes into a field, so a date field can hold text and a score field can hold a word. Objectives, criteria, and methodology usually live in separate tabs or files, connected only in the memory of whoever built the sheet. Multiple people working in the same file risk overwriting each other or working from a version that's already gone stale. And a spreadsheet doesn't talk to a force plate, a GPS unit, or an athlete management system. Every connection has to be manual, and manual is where consistency breaks.
